504 Plans
School Counselors are case managers for all 504 plans, which includes annual review meetings and frequent meetings one on one with the student. A 504 plan is a legal document that allows students an equal opportunity to participate in the school’s activities and programs. A teacher or parent can request a determination meeting for a student to qualify for a 504 plan and then the 504 team would gather. The 504 team consists of the student (if appropriate), the student's parent/guardian(s), and school staff.
Information is gathered regarding the student and during the determination meeting the following 3 questions must have a yes answer from the 504 team.
Does the student have a physical or mental impairment?
Does the physical or mental impairment substantially limit one or more major life activity?
Does the student need special education or related services to receive equal opportunity to participate in the school’s activities and programs?
If the answer is yes to these three questions then a 504 plan can be put into place and accommodations established by the students team of teachers, counselor, parent and administrator. Examples of students on a 504 plan, include, but are not limited to, students with diabetes, ADHD, or clinical depression. A 504 depends on the needs of the student.
